Synergy are seeking to employ a Qualified Social Care Team Manager to manage the Older Persons Physical Disability Team and ensure that effective assessment, care planning and case management systems are in place and deliver appropriate services to people with Physical disabilities in the context of their need, develop and commission services for individuals with Physical Disability and contribute to the Joint Commissioning process, take responsibility for the recruitment and management of all staff including supervision, performance monitoring and appraisal, discipline and development and ensure that all staff receive clinical/professional supervision through their employing organisation.
